Dr Poh Seng TAN graduated from the University of Melbourne in 2003 and obtained the Master of Medicine (Singapore) in 2007. He received further advanced specialist training in Gastroenterology & Hepatology and was certified by the Specialists Accreditation Board of Singapore in 2011. In 2013, Dr Tan further undertook his fellowship and research training in advanced hepatology and transplant hepatology at the Recanati/Miller Transplantation Institute and the Tisch Cancer Institute, Icahn School of Medicine at Mount Sinai (New York, USA). Dr Tan’s clinical expertise includes advanced liver diseases, liver cancer and liver transplantation, in addition to common digestive diseases and endoscopic procedures. He has authored or co-authored numerous research publications and is the principal investigator for ongoing clinical studies in the field of liver transplantation, immunosuppression and hepatocellular carcinoma.

Dr. Koh Tsingyi is a Senior Principal Clinical Pharmacist in Solid Organ Transplantation at the National University Hospital Singapore (NUH). She completed her Doctor of Pharmacy at the University of Illinois-Chicago and is a Board Certified Pharmacotherapist. She has practiced in the area of solid organ transplantation for over 10 years. Prior to joining NUH in 2013, she practiced as a Clinical Pharmacy Specialist in Solid Organ Transplantation at Boston Children’s Hospital where she cared for kidney, liver, intestine, pancreas and multivisceral transplant patients. She had been a reviewer of The AST Handbook of Transplant Infections and have co-authored a chapter on adverse drug reactions and drug-drug interactions. She is also a lecturer for the Doctor of Pharmacy Programme at National University of Singapore in the topics of transplant pharmacotherapy and hepatology. Her areas of interest include medication adherence and transplant infectious diseases.

She is a member of the expert panel that developed the International Consortium for Health Outcomes Measurement (ICHOM) Diabetes Standard Set and is currently President of the International Society for Pharmacoeconomics and Outcomes Research Singapore Regional Chapter. Dr Audrey Tan is a public health specialist with a masters’ degree in health promotion. She has been in the area of health promotion for more than 25 years.

Her current role in NHG’s Population Health Division includes supporting the Living Well Office with strategic planning and content development of upstream initiatives in the workplace and school settings.

One of her areas of interest is in health literacy as this is key in communicating health messages effectively to the target audience. She was in the team that modified USA’s test of functional health literacy assessment for Singapore. She has also conducted several workshops on writing for health literacy and plain language guidelines.

Ms Corrinne Tan graduated from NUS with a BSC Pharmacy Hons and subsequently obtained an MBA with Specialisation in Healthcare Management from NUS Business School. She also holds a Diploma in Retail Management from Kaplan University and is ACTA certified. She currently works in NHG Pharmacy as Assistant Director, Pharmacy Transformation Office, which project manages the NHG cluster-wide pharmacy initiatives and is also seconded to Chief Pharmacist Office, MOH as a Project Administrator to look into new care models with last mile medication delivery. Prior to NHG Pharmacy, Ms Tan was an Area Manager, Merchandising Manager (Health category) as well as head of the Pharmacy Department in Watson’s Personal Care Stores. Jye Chyi is practicing in Tan Tock Seng Hospital (TTSH) as a principal clinical pharmacist. She graduated with a BSc (Pharmacy) from NUS, and MSc (Clinical) Pharmacy from Queen’s University Belfast, and holds a board certification in geriatric pharmacy and pharmacotherapy.

She was one of the pioneers in the setting up of pharmacists’ home medication review services, the Pharmacist Outreach Program in 2010, and has worked with multidisciplinary teams to offer pharmacists’ home medication reviews. Jye Chyi has served in MOH workgroups, such as Centre-based Community Care Guidelines and Guidebook, Core Metrics on Incidence of Drug-Related Problems. She is involved in training diverse groups ranging from the public to healthcare professionals on medication management.

Jye Chyi’s other passion is in training and mentoring for the future. She has led the initiative to restructure various training programs within TTSH pharmacy for pre-registration pharmacists, junior pharmacists, pharmacy interns and pharmacy technicians.

Such responsibility requires her to take charge of the clinical training and professional development of more than 50 Pharmacy Technicians and Pharmacy Assistants. Adjunct Associate Professor Derrick Aw Chen Wee is a dermatologist and physician in Sengkang General Hospital (SKGH).  Prior to joining the Singhealth cluster, he was the head of dermatology division in the National University Hospital (NUH), Singapore.  As a senior consultant, his subspecialty interests are acne, psoriasis, eczema, urticaria, lasers for medical and cosmetic uses, minimally-invasive aesthetic procedures, cosmeceuticals and hospital dermatology. As the Clinical Education Lead for Medical and Associate Dean in SKGH, he overseas undergraduate medical training of students from all three medical schools in Singapore. In Yong Loo Lin School of Medicine, National University of Singapore (YLLSOM), he is the co-chair of the Collaborative Learning Cases committee, a student house Master mentor, and Associate with the Centre for Medical Education.  In contributing to postgraduate education, he sits in the Resident Advisory Committee for dermatology, the Singhealth Performance Review Subcommittee of SKGH, and the Singhealth Medicine Academic Clinical Programme (ACP) Education Oversight Committee.  With numerous teaching and achievement awards to his record, he has delivered more than 300 local and international lectures to medical students and residents, dermatologists, family physicians and other clinicians, pharmacists and the public.  He played an instrumental role in training the first advanced practice nurse in dermatology in Singapore as well as co-producing several nurse-led clinic initiatives in NUH.  He has written four books on clinical medicine (Look and Proceed, Armour 2002), clinical dermatology (Dermatology Picture MCQs, Armour 2005) and self-assessment in internal medicine (Modified Essay Questions for Medicine Finals Vol I and II, World Scientific 2012 and 2016).  He is a reviewer for several medical and nursing journals, Associate Editor for Journal of Dermatology & Cosmetology and sits in the Editorial board for Journal of Surgical Dermatology.  He has participated as principal investigator in phase three clinical trials on urticaria, atopic eczema and psoriasis.  His scientific publications exceed 60 and conference papers exceed 70.  He now sits in the Faculty of Medical Experts, Academy of Medicine, Singapore.  A firm believer in faculty development, he facilitates regular workshops and drives interprofessional education; a key interest of his is the written assessment.

Dr Cassandra Chang graduated with a Doctorate in Pharmacy (PharmD) from National University of Singapore in 2017. She is currently working at Singapore General Hospital (SGH), as an outpatient clinic pharmacist. She works closely with the Department of Urology in SGH with a multidisciplinary team caring for patients with prostate cancer. Catriona works as the Executive Director of the Irish Institute of Pharmacy (IIOP), based in the Royal College of Surgeons in Ireland (RCSI). Over the past five years, she has led the establishment of the IIOP, on behalf of the pharmacy regulator, and has overseen the successful implementation of new legislation in the areas of CPD and training for registered pharmacists in Ireland. In her earlier career she held leadership positions in pharmacy and business and has worked in academia, as an educator and researcher, in Trinity College Dublin. She is particularly interested in the areas of coaching, leadership development, team dynamics and reflective practice, and is currently leading the strategic development of lifelong learning at RCSI. Dr Camilla Wong has 25 years of experience in healthcare and is currently Director, Allied Health, Sengkang General Hospital (SKH). She is also Deputy Director, Group Allied Health, SingHealth (SHS), overseeing talent development, and is Senior Deputy Chief Pharmacist, MOH.

Recognised as a leader in healthcare, she has been appointed to numerous committees spanning a variety of portfolios including, the SHS Allied Health Council, Institute of Patient Safety and Quality and Patient Experience Council, the SKH IT Steering Committee and Medical Board, and the Singapore Pharmacy Council, MOH Pharmacist Specialist Accreditation Board, HSA Product Vigilance Advisory Committee and MOH One Rehabilitation Committee.

Dr Wong has been a role model and mentor to pharmacists and other healthcare professionals, and many have gone on to helm leadership and headship positions.

Kheng Yong leads the Singapore General Hospital's Asthma and Allergy Pharmacy Clinical Services, which include the Asthma Pharmacist Counselling Service, as well as the Sublingual Immunotherapy Service. He also runs the Medication Management Service, and Hemodialysis Disease Management Clinic. In 2018, he was awarded the Ministry of Health Health Manpower Development Plan (HMDP) fellowship to train in Asthma, Allergy & Clinical Immunology at the Alfred Hospital, Melbourne. Dr. Cook joined Duke-NUS Medical School in June 2006, and established Medical Education, Research, and Evaluation Department and helped establish the Academic Medicine Education Institute (AM.EI) launched in 2012.  AM.EI is a joint venture with Duke-NUS and SingHealth –designed to promote excellence in education for Health Professional Educators. In 2014, she was accepted into the NUS Teaching Academy Fellows and in 2016 received the Master Scholar Award from the International Association of Medical Science Educators (IAMSE).  She currently is the Deputy Head, Office Education and Deputy Director of AM.EI.

Ms Wong Yee May is a Cardiology Specialist Pharmacist at Tan Tock Seng Hospital, partially seconded to the Ministry of Health, Chief Pharmacist's Office to work on Collaborative Prescribing since 2015.  She was in the initial workgroup initiative, then the Standing Committee when it was formed in 2017, and involved in all three subcommittees (Curriculum and Education, Practice Standards and Competency, Clinical Governance).  As part of the Curriculum and Education subcommittee, she contributed to the development of the 12 to 14 week curriculum and implemented some changes through the cohorts.  Ms Wong continues to ensure day-to-day smooth running of the program, and upskilling of the Advanced Practice Nurse and Pharmacist trainee facilitators to take on some of the teaching in the program.

Dr Wang Aiwen is a drug information pharmacist at Singapore General Hospital (SGH), and has a joint appointment as a lecturer in National University of Singapore (NUS). Her responsibilities as a drug information pharmacist includes promoting evidence-based medicine practices through drug information consults, evidence-based reviews for the Pharmacy and Therapeutics Committee, and other research and educational activities within the Department of Pharmacy. Since 2008, she has been involved with a wide range of educational activities within the Pharmacy department at SGH, SingHealth, and even at the national level. Saw Yashi graduated from Bachelor of Pharmacy (Hons) from the National University of Singapore and received her Masters in Clinical Pharmacy from the University of Southern Australia. She is also a board certified pharmacotherapy specialist. In 2012, she completed a month of heart failure traineeship from the University of Utah, USA and had since then led and run a medication therapy management (MTM) clinic for cardiac patients in NUH. She was also awarded a MOH scholarship to pursue a one-year PGY2-equivalent pharmacy residency in cardiology from the University of Pittsburgh, USA in 2015. Presently, she is spending most of her time caring for patients in the cardiac coronary care unit in NUH.

Dr Wee Hwee Lin is joint Assistant Professor in the Saw Swee Hock School of Public Health and Department of Pharmacy, Faculty of Science at the National University of Singapore. She is a member of the expert panel that developed the International Consortium for Health Outcomes Measurement (ICHOM) Diabetes Standard Set and is currently President of the International Society for Pharmacoeconomics and Outcomes Research Singapore Regional Chapter. Dr Wee’s interest is in economic evaluation and health services research. Her recent work includes using dried blood spots for assessing adherence to statin.

Larissa is a board-certified geriatric pharmacist with more than 5 years of clinical working experience in Yishun Health. She works primarily with the geriatric population focusing on delivering clinical care for patients with dementia. In the outpatient ambulatory care setting in Khoo Teck Puat Hospital, she runs the Geriatric Pharmacist Clinic to optimise medication care management. She also participates actively in the 6th Vital Sign (Cognition) Delirium and Dementia Workgroup, to establish the framework for assessment and management of elderly patients with cognitive deficits. As the Inpatient-In-Charge of Yishun Community Hospital Pharmacy, Larissa oversees the pharmacy’s administrative and operational processes in the hospital.

Agnes Chan heads the Therapeutic Products Branch of the Health Sciences Authority (HSA), which performs the scientific evaluation for new and generic drugs approval as well as the post-approval variations throughout the products life cycle. Agnes graduated from the University College London and is a pharmacist by training. She began her career in HSA as a clinical evaluator, and had held various positions in the different functions of the Health Products Regulation Group, including the Deputy Director of Legislation and Director of Scientific Advisory Office. Mark is a Senior Regulatory Specialist in the Therapeutic Products Branch and has been with the Health Sciences Authority for the last 12 years. Trained as a pharmacist, his main work in HSA includes the clinical review of new drugs and label updates. He currently leads a team in the management of post-approval variations, including reclassification of medicines. He is also involved in various initiatives which aim to provide digital solutions for industry stakeholders to streamline business processes.

A/P Dennis Seow is the current Head and Senior Consultant of Department of Geriatric Medicine (GRM), SGH. He is the Lead Clinician for the GRM Memory Clinic Service and the current Clinician Lead for SGH on the ACP Steering Committee, Agency for Integrated Care.

He served as Programme Director for the SingHealth Services (SHS) Senior Residency Programme for Geriatric Medicine from 2012 till 2016. He holds Adjunct Appointments in Yong

Pei Shieen has a special interest in neuropharmacology. She works collaboratively with neurologists, nurses and allied health professionals in the Neuroscience community and takes the lead in optimizing pharmacotherapy. She is actively involved in teaching neuropharmacology topics in institutional and national training programmes for pharmacists and other healthcare professionals. She also serves as the Neuroscience Co-Lead for Singhealth Allied Health Research Committee and Singhealth Duke-NUS Neuroscience Academic Clinical Programme Allied Health Lead. Her recent new role as an educator in National University of Singapore (NUS), where she was an alumnus, allows her to contribute to the development of future pharmacists. Dr Chow Wai Leng is Director, Health Services Integration and Development division at the Ministry of Health, Singapore. She oversees the development and implementation of new community-based services as well as the integration of various care providers across the care continuum with the objective of developing an integrated ecosystem in the community that addresses both health and social needs of individuals. She also oversees the monitoring and coordinating of the strategy for community mental health and diabetes. Dr Chow is a family physician by training and has extensive experience in health services research. She previously headed the Health Services Research department in Eastern Health Alliance which focused on providing data and research findings to inform senior management decision-making and implementation research. Dr Chow area of interest is in developing and implementing new care models as well as the use of research and analytics to guide the management of patients and resource allocation.

Dr Ivan M. H. Woo is Principal Medical Social Worker with Tan Tock Seng Hospital, Faculty Member of the Centre for Healthcare Innovation Co-Learning Faculty and Consultant with the S R Nathan School of Human Development, Singapore University of Social Sciences. He is also a Certified Thanatologist with the Association for Death Education and Counselling and a Registered Social Worker with the Singapore Association of Social Workers. Valued for his opinions on health and social care, Ivan has been invited to be the Committee Member of HELP Family Service Centre, Member of the Tan Tock Seng Hospital Allied Health and Pharmacy Research Committee, and Reviewer for the National Healthcare Group Health Outcomes and Medical Education Research (HOMER) Grant and the Changi General Hospital Changi Health Fund. He has received several awards in recognition of his contributions to research, clinical practice and education, including the highly competitive Li Ka Shing Prize, given annually for academic excellence to the best of the elite students at the University of Hong Kong, and the Promising Social Worker Award, one of the highest accolades to be bestowed to a social worker in Singapore. Ms. Ma Yuet Ting received her Bachelor of Science in Pharmacy with Honours from the National University of Singapore. She is a Board Certified Ambulatory Care Pharmacist (BCACP).

Ms. Ma is a Senior Pharmacist at National Healthcare Group (NHG) Pharmacy, based in Toa Payoh (TPY) Polyclinic where she is active in her role in the Hypertension, Diabetes and Lipids Clinic (HDL-C). She is also the pharmacist-in-charge of the TPY Medication Clinic where pharmacists perform medication reconciliation for patients discharged from restructured hospitals and highlight drug-related problems to the doctors before consult. Together with other community pharmacists, she piloted Pharmaceutical Care Services (PCS) programme jointly initiated by the National Pharmacy Programme Management office (NPPMO) and Agency for Integrated Care (AIC) to help seniors and caregivers with any medication-related issues in the day care setting.

Reshma Lhode is a Senior Clinical Pharmacist at Guardian Health and Beauty. She received Board Certification in Geriatric Pharmacy from the US Board of Pharmacy Specialties in 2015. She has been working with nursing homes in Singapore since 2011. Reshma was awarded HPB Community Pharmacy Health Ambassador in 2010 and 2011, as well as Community Pharmacist of the year in 2012. Reshma has led the Colorectal awareness campaign or FOBT (Foecal Occult Blood Test) distribution at Guardian, in collaboration with the Singapore Cancer Society. In her role as a pharmacist in nursing homes, she provides medication management services such as medication review and audits. She also assisted the homes in developing SOPs related to medication management. With this experience in handling elderly patients, she has been spearheading Guardian’s patient care activities over the past years. Her passion for eldercare continues to propel her to take up leadership roles in this area.

Choo Yan Cheng serves as a community pharmacist in Singapore. His day-to-day job in a heartland area of Singapore gives him great exposure to customers from an elderly population. As part of the Intermediate-to-Long Term Care (ILTC) Pharmacist Workgroup, he works with various long term care settings including nursing home, welfare home and dialysis centre. He received Board Certification in Geriatric Pharmacy from the US Board of Pharmacy Specialties in January 2019.

He presently serves in the Pharmaceutical Society of Singapore's ILTC Workgroup as Chairperson. He has a personal interest in expanding his involvement in community care to assist with medication management for the elderly in his capacity as a healthcare professional.
